There are many web-based proxies, are you looking to download a script and host it on your own server? If so there are two that would really get your attention:

PHProxy - Download from: http://webscripts.softpedia.com/script/Internet-Browsers-C-C/PHProxy-33336.html

Glype - Download from:
http://www.glype.com/downloads

I prefer Glype, as you can download the proxy and then an additional plugin that will get YouTube videos to work, both of which are completely free.
