When a virgin has her first sexual intercourse it can but it doesn't have to be the case that blood is coming out of her vagina. This can be the case when her hymen or 'maidenhead' has been damaged by the penetration. Some girls have a very small hymen or a really strong one so that it doesn't brake when they have sex for the first time. Also the hymen can be damaged by playing sports, using tampons, or when the girl is masturbating.

So yes, blood CAN come out when the hymen breaks during the girl's first sexual intercourse but it doesn't mean anything if that is not the case regarding to her virginity.
